IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
In compliance with the Affordable Care Act (ACA), Atlas Excavating, Inc. is required to
provide individuals with Form 1095-C (Employer-Provided Health Insurance Offer and
Coverage), which outlines your health insurance coverage information for the prior
calendar year.

Note: You do not need this form to file your federal income taxes. The information may
assist you in preparing your return, but it is not required. You should keep a copy for
your records if you request one.

Request Process
Forms 1095-C are available upon request. If you would like to receive a copy of your
applicable form, please contact us using one of the following methods:

• Mail: Submit a written request to 4740 Swisher Road, West Lafayette, IN 47906

Atlas Excavating, Inc. will provide your requested form within 30 days of receiving your
request.
